Yes: Call in sick. Follow CDC, WHO, and FAA guidance. Contact your LEC rep and CPO to advise status. If diagnosed with COVID-19, you will be pay protected for the value of trips missed or credit lost as outlined in LOA #20-01.
2. Has a member of your household been diagnosed with COVID-?
and/or
3. Have you been directed to quarantine, receive medical evaluation or treatment in relation to COVID-19 by any company, medical, or government personnel or legal order?
No: Continue to question 4.
Yes: (to 2 and/or 3): Quarantine as directed and follow CDC, WHO, and FAA guidance:
-First, contact a doctor or QHCP and document all results.
-Next, contact your LEC rep.
-Then notify the company via CPO and Crew Scheduling to be removed from future rotations.
-Continue to maintain documentation of all visits to health-care professionals for pay treatment as outlined in LOA #20-01.
4. Do you have other concerns related to COVID-19, such as yourself or members of your household who have been exposed to or are at a high-risk of infection?
No: Use good judgment and follow guidance from the CDC, WHO, FAA, and the company on how best to protect yourself while working a rotation.
Yes: Consult a doctor or QHCP, document results, then contact your LEC rep and CPO with case findings.
Answer the following:

No: Continue to question 2.
Yes: Call in sick, then:
-On a domestic rotation: Contact the company nurse line, aka Air Crew and Passenger Health Services (ACPHS), at 404-715-2324.
-On an international rotation:
-Follow CDC, WHO, FAA, and company guidance.
-Any pilot outside the United States with concerns about their health or an illness should immediately contact MedAire at 480-333-3800 or 480-333-3595, or through the duty pilot at 888-676-7737.
-Refer to the FOM for further guidance.
-Contact your LEC rep.
